Enrollment
Enrollment into the Nationwide Program can be done through an FBC
approved Retirement Specialist or
on your own.
To
self-enroll:

1. |
Download
the Enrollment Kit from the
Plan Forms
page. Detailed
instructions for each form are included at the top of each
page.

2. |
Complete
and submit the Salary Reduction Agreement (SRA) to your
payroll office. Fax a copy to the FBC (see fax number
on form). |

3. |
Complete
and submit the Maximum Contribution Worksheet to your
payroll office. Fax a copy to the FBC (see fax number
on form). |

4. |
Complete
the Nationwide Enrollment Form and fax to the FBC (see fax
number on form). |
To
enroll with the assistance of a Retirement Specialist either:

the first page to the address on the form to have an advisor
