Enrollment

 

Enrollment into the plan can be done through a financial advisor or on your own.

 

To self-enroll:

  1.

Download the Enrollment Kit from the Plan Forms page.  Detailed instructions for each form are included at the top of each page.

  2. Complete and submit the Salary Reduction Agreement (SRA) to your payroll office.  Fax a copy to the FBC (see fax number on form).
  3. Complete and submit the Maximum Contribution Worksheet to your payroll office.  Fax a copy to the FBC (see fax number on form).
  4. Complete the Nationwide Enrollment Form and fax to the FBC (see fax number on form).

 

To enroll with the assistance of a financial advisor either:

  A.

Phone or email the financial advisor for your district.  Click here to find the advisor for your district.

  B. Or download the Enrollment Kit from the Plan Forms page and mail in the first page to the address on the form to have an advisor contact you.
